Can a birth mother be removed from their childs birth certificate?

Can you answer shortstuff’s question about Adoption?:

Could the birth mother of a child be removed from the birth certificate, and could you add a nother person as mother?

    Adoption Feedback: That’s exactly what happens in an adoption. The birth mother has to terminate her parental rights in order for it to occur, and a new birth certificate has to be issued.

    It can’t be done randomly or out of spite, though.

    Adoption Feedback: No, they cannot. People can be added, not taken away. That’s why it’s a birth certificate – it’s a record of the birth.

    Adoption Feedback: yes. That is what they did for my two adopted children. I personally think it is wrong. It is a “birth” certificate and I was not the one who gave birth to them.
